HOW CAN I COMPARE MY OPTIONS?
You can compare our plans and the Original Medicare Plan using a Benefit Overview and a Summary of Benefits. The charts in this website list some important health benefits. For each benefit, you can see what our plan covers and what the Original Medicare Plan covers. Our members receive all of the benefits that the Original Medicare Plan offers. We also offer more benefits, which may change from year to year.

Not sure which plan best suits you? Since everyone’s health needs are different, we offer a variety of Medicare Advantage Plans, including Medicare Prescription Drug coverage, so you can choose the one that fits you and your health needs. There are a couple of ways you can compare our plans:

  • By choosing a county to compare plans available within your area Select County
  • by which Health Benefit is most important to you.
  • By Plan Name if you already know which plan you are interested in.

WHO IS ELIGIBLE TO JOIN?
You can join if you are entitled to Medicare Part A and enrolled in Medicare Part B and live in the service area. However, individuals with End Stage Renal Disease are generally not eligible to enroll in the Gold, Platinum or Emerald Plan unless they are members of our organization and have been since their dialysis began.

CAN I CHOOSE MY DOCTORS?
As a Medicare Advantage HMO in Florida, you must stay in our network of providers. We carefully and continuously evaluate our network of physicians and other healthcare providers so we can offer the quality care that you deserve. Our network is quite extensive

DOES MY PLAN COVER MEDICARE PART D DRUGS?
As a member of the GOLD, PLATINUM, and EMERALD plans, you will receive prescription drug coverage. You can only join one Medicare Prescription Drug Plan.

Why Medicare Advantage?
Original Medicare pays for many health care services and supplies, but it often leaves over 20% for you to pay. In addition, there is limited drug coverage. That's why most people with Original Medicare carry additional Medicare coverage from a private insurance company - at an added premium cost (call Medicare Supplement Policies or Medigap Policies). Many people instead choose to receive coverage under a Medicare Advantage (MA) Plan. With a MA Plan, you receive more coverage than Original Medicare and don't need a Medicare Supplement policy.

Optimum HealthCare is a health Plan with a Medicare contract. Medicare approved MAPD HMO plans available to anyone entitled to Part A and enrolled in Part B of Medicare through age or disability (for MA plans, individuals must have both Part A and Part B). Medicare approved HMO Special Needs Plans (SNPs) available to anyone who meets the specific eligibility requirements of the SNP and is enrolled in both Part A and Part B of Medicare through age or disability. To qualify for a Dual Eligible SNP (DSNP), you must also be eligible for Medicaid assistance from the State. Premium for the DSP and copayments/co-insurance for Low Income Subsidy eligible beneficiaries may vary based on income. Enrollment period restrictions apply. Call the plan for details. You must continue to pay your Medicare applicable premiums if not otherwise paid for under Medicaid or by another third-party. Plans may be renewed annually. All plan types may not be available in all areas. Copayment and authorization rules may apply
