Optimum HealthCare is a health plan with a Medicare contract. You may enroll in some plans only during specific times of the year. For more information, please contact the plan. You are eligible for membership in our plan as long as: 1) You live in our geographic service area; 2) You are entitled to Medicare Part A; 3) You are enrolled in Medicare Part B; 4) You do not have End Stage Renal Disease (ESRD) (with limited exceptions) and 5) For our Special Needs Plans, you meet the special eligibility requirements. Please contact us to obtain more information. You must continue to pay your Medicare Part B premiums (or balance of Part B premiums on Part B premium refund Plans) even if the Plan premium is zero. Premiums, co-pays and co-insurance for Dual Eligible Special Needs Plans mayvary based on the level of help that beneficiaries may receive from Medicaid. For full-benefit dual-eligible members, the Part B premium could be paid by the State. Benefits, Formulary, Pharmacy network, Premium and or co-payments / coinsurance may change on Jan: 1 2013. People with limited incomes may qualify for Extra Help to pay for their prescription drug costs. If eligible, Medicare could pay for up to one hundred (100) percent of drug costs including monthly prescription drug premiums, annual deductibles, and co-insurance. Additionally, those who qualify will not be subject to the coverage gap or a late enrollment penalty. Many people are eligible for these savings and don’t even know it. For more information about this Extra Help, contact your local Social Security office or call 1-800-MEDICARE (1-800-633-4227), 24 hours per day, 7 days per week. TTY users should call 1-877-486-2048. On Dual Eligible Special Needs Plans premiums, co-pays and co-insurance may vary based on the level of help that beneficiaries may receive from Medicaid.
