Case Management is a free service offered to all health plan members. We want to help you stay as healthy as possible. We will also work with you to prevent complications.
Case Managers are nurses who can help you manage your illnesses and conditions. They do this through:
- Coordinating services with your doctors, especially your Primary Care Physician (PCP)
- Assisting you to better manage your condition(s) at home
- Providing education on issues affecting your health
- Providing resources to help support your wellbeing
Periodically, a case manager will call you to review your health status. Together, you and the nurse will work to achieve your health care goals.

Participation in Case Management is voluntary and does not alter your benefits.
