Disease Management Programs

The Plan's Disease Management Programs provide support for members with chronic conditions such as Diabetes or Heart Disease. The service is included with all benefit packages and is provided at no additional cost to the member.

Information is gathered from the member a number of ways such as the returned Health Assessments, claims data, and laboratory history. This allows the Plan to determine the members most likely to benefit from the program. The Disease Management support depends on the member’s understanding of the condition, ability to avoid or reduce complications and willingness to make some lifestyle changes, if needed.

The Disease Case Manager is a nurse who works with members to answer questions, provide education and assist in resolving problems such as medication issues. The Disease Case Manager will develop goals and steps the member can take to improve health. The nurse communicates progress with the member’s Primary Care Physician (PCP).
